Onset: This patient nail involution, involution is where the nail curved into the sides of the nail. Normally, we would see patients with this degree of involution as children, however this gentleman was lucky to not have a problem before. However, after an injury and an attempt at “self surgery”, the problem was not going away.
Presentation: He presented with signifiant pain as without regular treatment there is considerable pain as the nail grows into the skin, and because of the self surgery there was clearly a spike of nail causing all the inflammation and swelling.
Timeframe: 3 from First presentation to Nail surgery
Goal: Regain ability to enjoy daily life without being disrupted and delayed by pain.
Treatment: The permanent solution to this was to have a nail surgery procedure, so we booked in the nail surgery and did two conservative nail resections to keep him going until his desired surgery date.
We carried out the nail surgery, removing the side of the nail that was problematic and a reoccurring problem.
Outcome: The patient is now recovering but able to do a lot more on his feet. Despite best practice advice, he is comfortably walking more than 5 miles a day, which he started 2 days after the surgery. This photo is from a week after the surgery, he will be recovered in less than 8 weeks.
